Originally the name of a feather quilt bedding cover that was meant to be the only layer above you. Warm, snug, doesn't tend to get wraped in itself. Much more comfortable than sheets.
Americans, specifically Pukesick, may know them as Comforters. But Duvets tend to be softer, more lofty, generally nicer.
