When used as a noun, it refers to a cigar (or maybe a cheroot) made by the Italian monopolist state-owned tobacco company Monopoli di Stato.
There are different kinds, among which the Antico Toscano is the best (and more expensive).
Notice that the toscano is an unusual cigar: it is pointed at both ends. You cut it in two at the center, and light the cut surface - which means that every toscano is actually two cigars.
The toscano is a bit more dense and hard than the usual Cuban cigars. Don't worry, it is OK like that.
A little known fact is that the price of toscano cigars is imposed by law.
