One of the more uncomfortable idioms of Western usage. It is meant to denote a particularly weighted or heavy pause - perhaps after a revealing exchange in a conversation. The reasoning might be that pregnancy is fraught with anticipation of what comes next - or even that the moment will figuratively give birth to a new development.

It makes me think of huge women and VCRs.
